Add Dental Questionnaires

Dental Questionnaires enable you to quickly gather the dental details of a patient. You can provide patient specific questions and the options for the patient to respond to the questions. Dental questions are template specific and can be modified by the practice.

All offices associated to a practice group is displayed under the default template. You can create templates and move the offices from the default template to the new template. The dental questionnaire created is visible only to the specific template.

  1. Launch the Dental Questionnaire Setup window by navigating to Setup>Medical Setup>Dental Questionnaire.
    Dental Questionnaire Setup window opens displaying the list of templates in the left pane.

    The middle pane shows the list of dental questions associated with the template selected in the left pane. You can use the chevrons in the right pane to move the questions up or down the list.

  2. To create a template, click Add New Template button on the Medical Alert Setup window.
    The New Template Setup window opens.
  3. Type the name of the template.
  4. Select the desired offices from the Available Office list, and click the right arrow to move them to the Included Office.
  5. Click Apply.
    A confirmation appears indicating, recapturing all the health histories of the patient from the selected office.
  6. Click Ok continue.
    The Add/Edit Dental Questionnaire window opens.
  7. To set up the questions for the Dental Questionnaire, do the following:
    1. Click the Add New button (or select an existing question and click the Edit button).
    2. Select the Header? check box to use the description as a category header instead of as a specific question.
    3. Type the description as it appears on the questionnaire window.
    4. Choose the desired question type.
  8. Click Save.
    The new template appears on the Dental Questionnaire Setup window.
  9. For a preview of the questionnaire window, click Proof/Finalize.
