Appointment Intake Forms

The Appointment Intake Form is a screening form with health status questions that can be completed before an appointment.

To access the appointment intake form from the scheduler, right-click an appointment and select Appointment Intake Form > Update. To send an email with the appointment intake form to the patient, select Appointment Intake Form > Email. To access the form from the list view, click and select Appointment Intake Form > Update. To send an email with the appointment intake form to the patient, click and select Appointment Intake Form > Email
