Questionnaires

The Medical and Dental Questionnaires offer a robust way of gathering a patient’s medical information since the questions can be specific and there are additional options for responding, including text responses, dropdown options, and dates.

This information is completed in the new patient process and is accessible any time via the Patient Overview window, but the information is not automatically displayed. Both a sample medical and dental questionnaire are included that can be modified as needed by the practice.

Launch the Questionnaire Setup window by selecting Medical Questionnaire or Dental Questionnaire from the Medical Setup section of the Setup menu.

Like the Medical Alerts window, the left section shows the items listed with the header categories in an approximation of the medical information window. Individual items can be moved up or down the list using the up/down chevrons on the right side. Click the Proof/Finalize button at any time to see a mock up of the Alerts window. Be sure to do this at the end to save any changes.

To setup questions for the Medical or Dental Questionnaire:

  1. Click the Add New button (or select an existing question and click the Edit button.)
  2. Complete the box as desired

    1. Click the Header? checkbox to use the description as a category header instead of as a specific question.
    2. Type the description as it will appear in the Questionnaire window.
    3. Click the radio button underneath the desired question type.
      • A checkbox is a positive/negative response similar to the medical alerts.
      • A text box provides a single line for a text response.
      • A date gives a 00/00/0000 option for a response.
      • A memo provides multiple lines for a text response.
      • The Yes/No choice gives a dropdown with the Yes/No options.
  3. Click the Save button when complete
  4. Click the Proof/Finalize button to review the mock up of the questionnaire window.
