Chart Details

The Chart Details tab (and the lower section of the Tooth Chart tab) provides a more detailed, historical narrative of the patient's clinical relationship with the practice. The conditions and treatments displayed in the chart are described in detail along with documents and clinical notes.

Chart Details (on either the Tooth Chart or the Chart Details tab) include the following:

• Date of entry
• Office
• User
• Treatment ID
• Group (if applicable)
• Phase (if applicable)
• Provider
• Service Code
• Description
• Service restrictions (if applicable)
• Tooth
• Surface
• Type of entry
• Amount (Fees)
